A mobile technology that can certify eligibility for benefits at the point of service in real time and/or track the visit activities of delivery of services in outpatient settings through its Electronic Visit Verification (EVV) features.

Premise

  • MCOs-ACOs and ACEs have a requirement to provide home health care services to those in their respective assigned patient base when needed. Participation by Medicaid patients in Managed Health Care Plans -- ACEs, CCEs, HMOs, MCCNs, MCOs -- continues to grow under the Rauner Administration and stood at 2.1 Million. As of May 15th, the program experienced a 100,000 enrollment increase over the month of April. Most of the Contracted Medicaid MCOs, as well as those with specific ICP and MMAI components, have entered into sub-contracts with Home Health Care Agencies to fulfill their duties to the ABD Medicaid Patient Population.

  • Tracking the delivery of services in the field is an important component to the maintaining/reporting of appropriate quality of services as well as prudent fiscal responsibility.

  • An interoperable, direct, electronic reporting system in real time is essential to managing a field based multi-location delivery system.

  • As the Accountable Care Act requires the delivery of health care services to the spectrum of eligible persons in an out-patient setting; cost conscious, reliable, technology based reporting to better manage such delivery is essential to a fiscally well managed business.


Utilizing AccurateID For a More Efficient and Fiscally Responsible Home Healthcare/Care Coordination Delivery Program

  • Electronic Visit Verification (EVV) is required under recently enacted legislation, Public Act 097-0689, also known as the Save Medicaid Access and Resources Together (SMART) Act, Sec. 5-5f., sub-part (g) for Home Care.

  • CMM’s AccurateID has been approved by the State Department of Aging as a qualified EVV services vendor.

  • Code Marketing & Management, Inc.   • The relationship between CMM and Microsoft offers the advantage of Microsoft’s broad state-of-the-art technology, including its new Azure Cloud, creating highly-available, scalable applications and services using a rich Platform as a Service (PaaS) environment and supporting advanced multi-tier scenarios, automated deployments and elastic scale. Use of the Microsoft Azure Cloud affords infinite capacity and flexibility to fulfill any CMM application or program.

  • CMM and its Microsoft-certified partner provide a unique mobile solution using any smart-phone or compatible hand held device carried by a Home Health Care worker. The system monitors the location, timing and on-site delivery of home care, ensuring timely services and support for clients, streamlining the process and greatly reducing human error.

  • Provides anywhere availability of certain information on eligible covered persons in cases of disaster, trauma, accident or other unforeseen adverse situations is of great value in not only delivering the proper and appropriate service to people, but also in the saving of lives.


An EVV solution must be proactive as well as reactive.

Incorporating front-end, preventative solutions that track and report point of service care, Code Marketing & Management, Inc. has developed an interoperable application that offers a solution to this problem, incorporating 2D Code recognition technology, the Microsoft Azure Cloud and the CMM proprietary application, creating the management tool.

With the 2D Code in place on ID cards or other medium, such as a letter, a Home Health Care Provider, MCO, ACO or ACE would require service providers to scan the 2D Code when they arrive at the home or other place of delivery of service and scan the 2D Code again when they leave at the end of the session.

This action combined with the GPS transmission of a smart phone would provide the appropriate agency with the exact time and location of the service in real time, direct and free of intervention. 2D Code technology operates on all phone platforms and over 400 cell phones and hand-held devices.

All that is required is a smart phone or other hand held device with a camera and a data connection. In the event of no cellular access, the use of a traditional land line phone can be employed, utilizing existing manned systems. The 2D Code recognition software is a free Android & iOS application. The Microsoft Azure Cloud framework and application is provided by Code Marketing & Management, Inc.


How AccurateID Works

Each Home Health Care Employee is required to “scan” the 2D Code on an eligible recipient’s ID card. The 2D Code (personalized for each recipient) is scanned by the worker using any Internet capable and camera equipped phone at the point of service (e.g., the home) at time of arrival and departure.

Using tailored GPS and clock functions, CMM documents exact location and time of each visit, reporting proof of visit data to the provider to document reimbursement eligibility.

The application can deliver, per an agency’s specifications, including but not limited to, the following information when scanned:
  • Case Number
  • Vendor FEIN/SSN
  • Vendor Zip Code
  • Identification of the Provider Service Agent
  • Service From Date
  • Service To Date
  • Service Description
  • Quantity of Hours
And also:
  • Full name and current home address of the recipient of Home Care Services.
  • Customer ID.
  • Code ID - this is the individual 2D Code.
  • Scan Time - the Date and Time of the 2D Code Scan accurate to milliseconds.
  • Device ID - a unique identifier for the scanning device.
  • LNG- the longitude of the scanning device.
  • LAT - the latitude of the scanning device.
  • Customer Postal Code - the Postal Code (Zip) of the scanning device.
  • Name and phone number of customer’s next of kin.
  • Name and phone number of customer’s primary care physician.

Implementation

Simple and Easy

  1. Provider delivers a roster of eligible home health care recipients to CMM with name address, ID number, etc.

  2. Provider issues a specific ID (card, letter other) to participants bearing a printed 2D Code. The 2D Code (personalized for each recipient) is scanned by the worker using any smartphone camera at the point of service (the home) at time of arrival and departure.

  3. CMM will provide all collected data to the Provider and, if requested, to appropriate agencies (such as HFS or DoA) to facilitate monitoring as required by the Home Health Care Agencies.

  4. All information is server-based, utilizes Microsoft’s Azure Cloud technology and therefore secured:
    • Information is secure – meets HIPAA requirements
    • Can be updated, expanded/contracted or changed at any time
    • Information flow can be “turned on and off” at any time
    • Access can be secured through “PIN” or “Password”

Conclusion

The combination of the QR Strategies application with the Microsoft Azure Cloud technology creates an efficient, effective, secure and economical means and method to deliver specific information relative to timely delivery of services digitally and in real time to Home Health Providers, an important and useful tool in monitoring and managing its contracted home health provider, its personnel and deterring potential waste, fraud and abuse, streamlining the process and greatly reducing human error.
